Low Ferritin and Acne: What the Research Actually Shows

Short answer: low ferritin has not been proven to cause acne. A handful of small studies have compared iron levels in people with and without acne, and the results are mixed: some found slightly lower ferritin in acne patients that wasn't statistically meaningful, others found no difference at all. What is clear is that iron matters for healthy skin and tissue repair, that low iron stores are common, and that ferritin is easy to misread without context. It's worth knowing your number, but not worth guessing with supplements.

What is ferritin, and how is it different from serum iron?

Ferritin is the protein your body uses to store iron. A small amount of it circulates in your blood, and that level reflects how full your iron "savings account" is. Serum ferritin is the most useful single blood test for spotting iron deficiency (Daru et al., 2017).

Serum iron is different. It measures the iron moving through your blood at that moment, and it's easily pulled down by inflammation. Your body deliberately withholds iron from the blood when it's inflamed, through a hormone called hepcidin (Pasricha et al., 2021). So a low serum iron doesn't always mean you're short on iron, and a normal one doesn't guarantee your stores are full.

Ferritin can also run high for reasons unrelated to iron intake, such as fatty liver disease or alcohol-related liver disease (Kowdley et al., 2019).

One more wrinkle: there's no universal agreement on what counts as "low". Many labs flag ferritin only below about 15 ng/mL, the long-standing WHO cutoff, but some researchers argue that a threshold closer to 50 ng/mL better reflects iron deficiency in adults (Cancado et al., 2025). That means a result can be "in range" while your stores are on the lighter side.

Can iron deficiency cause acne?

Here's what the research actually shows, and it's thinner than social media suggests.

  • A 2023 case-control study of 114 young adults in Saudi Arabia found that people with acne had somewhat lower average ferritin and hemoglobin than people without acne, but the difference was not statistically significant. The one significant finding was more B12 deficiency in the acne group (Alharbi et al., 2023).
  • A 2013 study of 52 women with post-adolescent acne and 52 controls found no difference in ferritin, serum iron or hemoglobin, and no link between iron markers and acne severity (Balta et al., 2013).
  • A 1985 study of severe nodulocystic acne found low serum iron in 75% of patients, but ferritin was raised, not lowered. The author concluded that the low iron came from the body's response to skin inflammation, not from true iron deficiency. It returned to normal once the acne was treated (Leyden, 1985).
  • A 2024 genetic study using Mendelian randomization looked for a causal link between iron markers (including ferritin) and acne and did not report one (Wu et al., 2024).

So the honest summary is: there's no solid evidence that low ferritin causes acne, and the inflammation from severe acne can itself push serum iron down. If you've seen "low ferritin acne" threads on Reddit, those experiences are real to the people sharing them, but they aren't the same as proof.

Why iron stores still matter for your skin

Iron is involved in many processes in the skin, including oxidative stress and wound repair. A review of the research found that iron deficiency and anemia have been linked to slower wound healing, though much of that evidence comes from older and animal studies (Wright et al., 2014). Too much iron is a problem too: iron overload is linked to iron deposits in the skin and oxidative damage.

That's the balanced picture. Iron isn't an acne switch, but it's part of the recovery toolkit your skin relies on, and low stores can also leave you tired and run down in ways that make everything feel harder.

Who is most likely to have low ferritin?

  • People with heavy periods. Up to one-third of women of reproductive age experience heavy menstrual bleeding, which can lead to iron deficiency (Mansour et al., 2021).
  • Vegetarians and vegans. A meta-analysis of 24 studies found adult vegetarians had significantly lower ferritin than non-vegetarians, because plant iron is absorbed less efficiently (Haider et al., 2018).
  • Premenopausal women in general. Iron deficiency particularly affects children and premenopausal women (Pasricha et al., 2021).
  • People with conditions that affect absorption. Iron deficiency guidance recommends considering screening for coeliac disease as a possible cause (Pasricha et al., 2021).

Should you take iron for acne?

Not without testing first. Three reasons:

  1. Iron can build up. Hereditary hemochromatosis, one of the most common genetic disorders in people of northern European descent, causes iron to accumulate and can lead to liver damage (Kowdley et al., 2019). Adding iron you don't need is not harmless.
  2. Side effects are common. A meta-analysis of 43 trials found that ferrous sulfate, the standard iron supplement, more than doubled the odds of gut side effects compared with placebo (Tolkien et al., 2015).
  3. Low iron has a cause. Guidelines say an underlying reason should be looked for in anyone with iron deficiency (Pasricha et al., 2021). Taking a pill can hide the question of why your stores are low.

What to ask your doctor

  • Can we check ferritin, and read it alongside a marker of inflammation?
  • If my ferritin is low, what's causing it: periods, diet, absorption or something else?
  • If it's "normal", where in the range is it, and does that fit my symptoms?
  • If I need iron, what form and how long, and when should we retest?

Frequently asked questions

Can low ferritin cause cystic acne?

There's no good evidence that it does. In the one study of severe cystic acne, iron in the blood was low but ferritin was actually high, and researchers linked that to inflammation from the acne itself rather than iron deficiency.

What ferritin level is too low?

Many labs use about 15 ng/mL as the cutoff, but some researchers argue around 50 ng/mL is a better threshold for adults. Your doctor can interpret your number alongside your symptoms and other results.

Can I have low iron stores without being anemic?

Yes. Iron deficiency without anemia is common, and ferritin can drop before hemoglobin does. That's one reason ferritin is useful to check on its own.

Will fixing low ferritin clear my acne?

There's no research showing that. Correcting a real deficiency is good for your overall health, but acne usually has several drivers, so it makes sense to look at hormones, inflammation and other nutrients too.
